Chad Spain, MD, FAAFP: Chad Spain is a family medicine physician working for Intermountain Healthcare in Murray, Utah. He has a focus in Value Based Care, piloting a project for Intermountain in 2018, and now working as an Associate Medical Director for Castell Value Based Care. Outside clinical responsibilities, Chad is current President of the Utah Academy of Family Physicians, Associate Professor Adjunct Tract University of Utah DFPM, and board member of Castell Accountable Care. He enjoys running, Disneyland vacations, sports card collecting, and spending time with his family.

David B. Miner, MD. Dr. Miner is a Family Physician practicing in Salt Lake City, Utah.  Dr. Miner is the Medical Director for Maliheh Free Clinic (MFC).  MFC provides free, high-quality medical care to uninsured and impoverished members of the greater community.  MFC provides comprehensive medical care to an extremely diverse patient population.  MFC has over 80 volunteer medical providers and over 450 general support volunteers.  MFC provides community medicine training opportunities for Family Medicine Residents, Internal Medicine Residents, and Physician Assistant Students.

Dr. Miner came to Maliheh Free Clinic in March, 2018.  Prior to joining MFC, Dr. Miner was employed by Intermountain Healthcare where he practiced a full-spectrum of Family Medicine.

Dr. Miner and his wife, Kerry, have been married for thirty years.  They are blessed with five children, two grandchildren, and a fabulous dog.  Dr. Miner enjoys all sports.  He particularly enjoys triathlon, snow skiing, and boating.  Dr. Miner’s favorite hobby is reef aquarium keeping. Dr. Miner believes that the noblest aim in life is to strive to live to make other lives better
