Engaging Patients in Diabetes Care ECHO™ Series 

Join us for a six-part series to help you create a learning network  that will support and accelerate process and quality-of-care  improvements using health information technology, quality  measures related to diabetes and chronic kidney disease, diabetes  self-management education and support (DSMES), and  implementing diabetes process maps to improve patient outcomes.  There is no cost to participate. 

This series is designed for clinicians, care teams, caregivers and  management. 

Participants will be provided with a learning network through the  ECHO™ model that will support: 

  • How to engage your team to engage your patients
  • Ways to leverage telehealth and virtual services
  • Using tools and techniques to reach any patient
  • How to engage and refer to DSME 

Sessions begin April 7, 2021! 

All sessions will be held virtually on  Wednesdays from 12:15 p.m. to 1:15  p.m. MT and will run from April 7 to  May 5, 2021, with a follow-up call on  Aug. 4, 2021. 

A $500 stipend will be given to the  first five clinics to commit to  participate.

Outline and Learning Objectives
Description

This ECHO™ series is designed for clinicians, care teams, caregivers and management. The ECHO model™ will be
used to create a learning network that supports and accelerates process and quality-of-care improvements using
health information technology (HIT), quality measures related to diabetes and chronic kidney disease, diabetes
self-management education and support (DSMES), and implementing diabetes process maps to improve patient
outcomes.

Session 1: Introduction
Learning Objectives:
• Understand ECHO™ structure, project, roles and data

Session 2: Engage Your Team to Engage Your Patients
Learning Objectives:
• Team training and development
• Practice conversations with patients

Session 3: Leveraging Telehealth and Virtual Services
Learning Objectives:
• Discover the range of virtual services for individuals with diabetes
• Explore virtual self-management options
• Focus on diabetes and chronic kidney disease

Session 4: Tools and Techniques to Reach any Patient
Learning Objectives:
• Understand which patients are best suited to what kinds of techniques/resources

Session 5: DSMES: Engage and Refer
Learning Objectives:
• Understand curriculum of DSMES
• Identify which patients are best suited for the class
• How to help patients understand the value of the class for themselves

Session 6: Roundtable
Learning Objectives:
• Each clinic takes five minutes to share what you have changed in your process
