Overview
This will be a hybrid model conference, focusing on teen-to-adult healthcare transition. Topics will include aging out of public services, accessing services at colleges, and adaptive care plans. Lastly, patient awards and videos will be presented, where we will honor many teens and young adults who have gone above and beyond in caring for their health.
Objectives
- Discuss real life healthcare transition experience including tools available at public and institutional levels
- Identify resources for healthcare transition and the best practices for their usage
