Survey Request

My name is Scott Khuu and I am a third-year pharmacy student at the University of Utah collaborating with Elizabeth Bald (PharmD), Katherine Hastings (MD), and Stacey Slager (MS) on a research project involving the use of personal continuous glucose monitoring (P-CGM) devices for patients with type 2 diabetes in the primary care setting.

We are writing to ask for your help with a survey we are conducting aimed at identifying key barriers and facilitators for utilizing P-CGM for patients with type 2 diabetes. The survey should take ~5 minutes of your time to provide important insight from your perspective as a provider. The survey is due in three weeks on February 9, 2023.

The survey is confidential. Your individual answers will not be linked with you in any reports of the data. Your participation is voluntary and if you come to any questions you prefer not to answer, you are welcome to skip it and go on to the next.

At the end of the survey, there is a link that will direct you to a separate page to leave your contact information to be entered in a random lottery for a $100 Amazon gift card in compensation for your time.
